For good and valuable consideration, (receipt of which I acknowledge), I confirm that: *

I grant permission to use the video footage: 
in the church video ‘Sunday Supplement’ which will be available for anyone to view online on the StJ s website and via social media, including Youtube and Facebook. The video footage will also be stored on a single hard drive for editing purposes, pertaining to ‘The Sunday Supplement’ and other videos prepared by the St Js Group

I understand that the video footage may be edited, copied, modified, exhibited, published or distributed and waive the right to inspect or approve the finished product.

I waive the right to any royalties or other compensation arising or relating to the use of the video footage.

This Agreement and any dispute or claim arising out of or in connection with it or its subject matter or formation (including non-contractual dispute or claims) shall be governed by and construed in accordance with the law of England and Wales. Each party irrevocably agrees that the courts of England and Wales shall have exclusive jurisdiction to settle any dispute or claim arising out of or in connection with this agreement or its subject matter or formation (including non-contractual disputes or claims).

I have the right to: *

withdraw my consent to the St J's Group using and storing copies of the video footage for the specified purposes at any time;

withdraw my consent to the St J's Group storing my contact details on its database;

access the video footage, and any information the St J's Group holds about me at any time; and

request that the video footage and any information the St J's Group holds about Video Footage, and any information the Business holds about me be 
removed from any materials, future materials and internal databases by contacting the St J's Group at this email address: office@stjsgroup.church.
